This form is for any patient coming in for a procedure. This form is for any patient coming in for a procedure. The $10 pain package is a shot that lasts for 24 hours. The $31 pain package is the shot that lasts for 24 hours as well as medication for three days after the procedure. The medication used for pain is also an anti-inflammatory.
