At Baker Animal Clinic, we offer patient forms online so you can fill them out in the convenience of your own home or office. Completed forms can be emailed to us at baker-ac@hotmail.com, or you can print and bring them to your visit at the clinic.

Pain Package Agreement

This form is for any patient coming in for a procedure. The $34.00 pain package is a shot that lasts for 24 hours. The $70.50 pain package is the shot that lasts for 24 hours as well as medication for three days after the procedure. The medication used for pain is also an anti-inflammatory.

Kittens and Puppies

How exciting! Your new kitten or puppy has finally arrived and you want to do the best for them. When you come to Baker Animal Clinic for the first check up you’ll be greeted by our receptionist. She’ll help you take your new pet’s weight on our digital scale. Our veterinary assistant will escort you into the exam room and start your pet’s first exam with a temperature reading, a heart rate reading and get a brief history. Then Dr Maria will start at your pet’s nose and perform an oral exam, check for defects in their eyes, check ears for mites or disease, check for swollen lymph nodes, abdominal problems, hernias, quality of heart and lung sounds, skin and coat condition and body condition.

After that she’ll discuss housebreaking, crate training, feed management, heartworm prevention, flea and tick control, vaccination schedules, neutering, wellness exams and answer any questions you may have. Before you leave your pet will receive her first heartworm prevention whether Advantage Multi for kittens or Sentinel for puppies.

Dr Maria prefers you to come with many questions. She believes an informed pet owner makes the best decisions.
